Public Announcement for School Meals 

Special Provision Options (Provision 2 Non-Base Year & Community Eligibility Provision)

The Candor School District today announced an amendment to its policy for serving meals for children served under the National School Lunch Program and School Breakfast Program for the 2023-2024 school year, which would allow for all children at all schools/sites to be served meals at no charge.

Attention Seniors

If your student leaves the district or your student is a graduating senior, you can request a refund for their meal account by contacting Michelle Dougherty Email Michelle Dougherty.  Parents of seniors have one year to request a refund.  Please note that the food service point of service system balance is not synced with MySchoolBucks.  Thank you.

Meal Modification Information

Does your child have food allergies or require some form of meal modification?  Please select the appropriate choice below and complete the necessary documentation.

Complete One Form:

Parent or guardians must choose one of the following forms:

Request for Meal Modification form (must include doctor's note)

or

Meal Modification Medical Statement form (completed by healthcare professional)

Submission:

Once the chosen form is completed, it must be returned to the school nurse at the child's school.

Important note:  Completing both forms is unnecessary - only ONE is required to initiate the meal modification process.

The completed AD-3027 form or letter must be submitted to USDA by:
